Cornerstone Care is a non-profit network of federally qualified community health centers. For over 33 years we have grown in response to community health care needs. Our mission is to improve the health of our patients and the residents of the communities we serve. Our goal is to make affordable primary health care readily available to the entire community regardless of social or economic standing, or personal or demographic characteristics. From the child whose parent faithfully brings him or her to regular well visits to the retired farmer who may not have seen a doctor in decades, our health care professionals take care of patients from a wide range of backgrounds and circumstances. From the full range of chronic conditions to everyday acute problems, our providers are immersed in community health care.
Cornerstone Care served 26,190 patients, involving nearly 92,000 visits in 2011. Patients come from all over the tri-state area.
Cornerstone Care comprises nine locations throughout southwestern Pennsylvania. Each office is strategically located to promote access to primary health care for our communities of rural, small town and small city populations. Services vary by location. Cornerstone Care also operates a mobile medical/dental unit. Cornerstone Care Primary Care Center of Mount Morris is the closest center to Mon General Hospital and serves as the Teaching Health Center in the Family Medicine Residency Program. All Cornerstone Care locations are within easy reach of the Residency's teaching home.
Cornerstone Care spans a wide range of primary care.
Medical Services include family practice, pediatrics, OB/GYN, podiatry, chiropractic, massage therapy, laboratory services, radiology, and occupational medicine.
Psychiatry & Counseling Services include general and child psychiatry, family and individual counseling, group therapy, and psychological testing.
Dental Services include general dentistry and orthodontics.
Patient Support Services include prescription assistance programs, special pharmacy programs with local pharmacies, healthy women project, diabetes education, child literacy and a sliding fee discount program for uninsured patients.
Mobile and Outreach Services include cervical and breast cancer screening and education, oral health outreach and education, employment-related services, and mobile dental and medical care.
Cornerstone Invests in Innovation
Cornerstone Care has invested over $4 million over the last three years in new facilities and renovations, technology and connectivity upgrades and equipment.
Cornerstone Care is implementing electronic health records across all services. We engage in continuous quality improvement efforts and are currently working on national certification for the patient-centered medical home model.
